Income Verification Forms Urgently Requested

We would like to remind parents of the importance of completing and returning an Income verification form. The income verification form is pink and was included in your child's enrollment packet which was sent home during the first week of school.

We are encouraging parents to fill out the form as completely as possible and return it to school as soon as possible. It is very important since a good deal of our school and district funds are determined by the information we collect from our families.

We encourage you to contact us if you need a new copy of the form or if you have any questions about how to complete it. We are here to help. Please call us at 782.7100 if you have any questions.
